Please note the required forms needed to complete registration; 

  • most recent IEP, 504, or Psychological Evaluation
  • Camper Health History Form
  • Copy of most recent physical exam. 


Aside for the above required forms, there are additional forms needed to complete the enrollment process into our camp. 
Those forms will be added to this page at a later date.
 
Please note, we must have all required forms to complete enrollment in our office by June 5, 2021.
