Filling out this form starts the enrollment process of attending our program. Once the enrollment form is completed and the one time enrollment fee of $70 is paid, the participant’s registration with our program is secured.  You will receive an emailed invoice totaling the required amount due for enrollment, based on the options you choose below. Invoicing for our program is done on a monthly basis and must be paid in full in order for individuals to participate in the upcoming months activities.

Please keep in mind the fees only covers general programming, supplies, basic community outings like bowling, movies, etc. that cost under $8 per person. Any specialty community outings costing more will be the responsibility of the individual should they choose to participate. Notification will go out in advance of all specialty community outings requiring extra funding.
